What does this setting do?
Switches radiation in the contaminated areas on or off. On means anyone entering those zones without protective gear takes damage and needs treatment afterwards. Radiation therefore acts as a gatekeeper — it locks rewarding areas away from players who are not equipped yet.
Switching it off opens those areas to everyone. That makes the server more accessible but devalues protective gear, iodine tablets and the whole preparation such a trip involves.
Which value suits my server?
On if protective gear is obtainable. Otherwise off.

What to watch out for
Hazmat suits, masks and anti-radiation medicine then sit uselessly in the loot pool. If you disable radiation, reconsider their availability too.
